You will have received an email with your clinic appointment dates and times and we will send you a reminder SMS/text.

Please click on your local Momentum site to view the Participant information Sheet. You do not have to sign this document now, we will go over it with you at your first clinic visit and answer any questions you may have.

Download the
e-diary app

During the study we will ask you to keep a health diary using an app on your mobile device such as a mobile phone, tablet/ipad or lap top. Click on the button below for instructions on how to download the e-diary app onto your mobile device. Please bring your device to your first clinic visit and we will help you set up your user name and log in and show you how to use the diary.

Bring these items
to your first clinic visit.

  1. ID e.g. License, passport, library card.
  2. Current medications and supplements or prescriptions.
  3. Mobile device or smartphone. Please download the eDiary in step 3 prior to your appointment.
  4. Bank account details so we can reimburse you for time and travel.
